HFitness Study 2: Impact in Men Following Prostate Removal
HFITNESS produces significant improvements to incontinence and quality of life measures in men undergoing prostatectomy. These benefits are long-term, with improvements seen through 1 year post-operatively.
Background
The study addresses barriers to pelvic floor muscle training (PFMT) access, including limited therapist availability and financial constraints. Researchers developed HFITNESS—an online program delivering comprehensive long-term PFMT, pelvic floor education, and dietary/behavioral modification guidance.
Methods
Patients anticipating robotic-assisted laparoscopic prostatectomy (RALP) participated in HFITNESS rehabilitation. The 12-month program comprises three phases with multiple exercises featuring varied contraction types and durations, plus comprehensive dietary and behavioral technique education.
Assessment occurred at 3 weeks, 3, 6, and 12 months post-RALP using:
- International Consultation on Incontinence Questionnaire Male Lower Urinary Tract Symptoms (ICIQ-MLUTS)
- Incontinence Impact Questionnaire (IIQ-7)
- Additional satisfaction and daily pad use items
Results
The analysis included 21 men (64±6 years). At six-month follow-up:
- Significant improvements in incontinence degree and quality of life versus 3-week baseline
- Patient-reported improvement averaged 7.42±0.74 on a 10-point Likert scale
- 100% found the program easy to use, educational, and recommendable
- 89% expressed satisfaction
- Zero reported access problems or adverse events during interviews
